This is a 42-year-old lady who has a history of significant trauma to her nose. She had repair of facial fractures and multiple reconstructive procedures in the past. She was unhappy with her flat nasal tip, which she felt was sagging. She also had a septal deviation and nasal airway obstruction. She underwent secondary rhinoplasty, using cartilage and bone grafts from her ribs to open her airway and improve the appearance of her nose. She had an excellent postoperative result.

This patient had a history of a broken nose in the past, but also felt that his nose was “too big”. He desired correction of the hump on the dorsum of the nose and the downward plunging tip, which gave his nose a very long appearance. He also felt that his nostrils looked excessively large. He underwent an open rhinoplasty for correction of his post traumatic deformity, reduction of the dorsal hump and downwardly rotated tip.

This beautiful young lady had a history of trauma to the nose and mid face in the past. The nasal dorsum and tip were broad and flattened. She underwent open rhinoplasty with placement of a Medpor nasal dorsum prosthesis and tip rhinoplasty to raise and define the dorsum and the tip of the nose.

This patient wanted to change the appearance and function of her nose. She felt that she had a hump on the dorsum of the nose, a downward turning tip, and felt that the tip ‘wiggled’ when she talked. She had a history of a broken nose in her youth. She underwent reduction of the dorsal hump, placement of a cartilaginous graft at the root of the nose, straightening of the septum with cartilage grafts and remolding of the cartilages of the tip. She has a very natural appearing postoperative result.
